Surveys

Family Day Home Agency Surveys 48 Kb

Please note the AELCS office is no longer compiling the family, staff and provider surveys.

Programs are still required to complete the surveys; however, they no longer need to submit them to the AELCS office.  It is expected that programs will compile the results of the surveys themselves and incorporate any feedback within their QEP.

Validators will note that they are completed when they conduct the site visit.

The family surveys are available in the following ten languages:

French
Polish
Ukrainian
Simplified Chinese
Traditional Chinese
Vietnamese
Spanish
Arabic
Punjabi
Urdu

If you require the family surveys in any of the above ten languages, please contact Diane Langner and she will send you a copy.

Proprietary Rights of Accreditation Materials

A program’s eligibility to become accredited and to maintain accreditation status is linked to the status of the operating license. Therefore, the materials that are issued to a program to become accredited are owned by the program. They are not owned by any one individual and must not be removed from the program when an individual leaves or when a program is sold. New directors and owners must have access to the program’s accreditation materials to complete their accreditation process or to demonstrate that they are maintaining the ACCAP Quality Standards.
